Since our goal is to ensure ultimate convenience, below, you can find the necessary patient forms below. Please feel free to download, print, and complete these forms prior to your appointment. Doing so will expedite your visit.

Follow-up Appointments

COVID testing is required for all patients, regardless of vaccination status.

Covid Test Options:

In-Office procedures: Pick up a COVID test kit from the office to return 5 days prior to your procedure. This is a saliva test that can be dropped in our test collection box outside of the office anytime between 7 am-5 pm. If your test kit is not in the box by the appropriate date your procedure will be canceled and rescheduled.

St. Francis procedures: Go to 2200 Northern Blvd Suite 100B Greenvale, NY 11548 Parking lot Section: D. If your procedure is scheduled on a Monday, you must go for your COVID test the Thursday prior. The lab is open from 9 am-2 pm, no appointment is needed. If your procedure is ANY other day, please call the office for details. (Please see SF COVID Map in attachments)

Northwell procedures: Please call 833-487-2273 option 2 to schedule a COVID test at a lab near your home/work. You will tell the scheduler the date of your procedure, and they will schedule you accordingly.
